>This is the ROS/MAP Data Use Certificate. To gain access to the ROS/MAP data this certificate must be submitted for approval. Data Use Certificate Submission Download the Data Use Certificate: (click on the download button next to the file name) Open it in Adobe Reader, and confirm your agreement with the terms of use by entering the required information, including electronic signatures. E-mail the completed form to ad.datausecert@sagebase.org. Challenge administrators will review the signed document and notify you by e-mail when it is approved. You will then be a member of the "Alzheimers Challenge RUSH Access Approved Team" and be able to access the Challenge data. You can view the list of approved participants from the "My Teams" section on your home page. Note that both you, as the Challenge participant, and a signing official must sign the Data Use Certificate. A signing official is defined as any person with the authority to oversee the participant?s activity with respect to the Challenge (i.e. Department head, school principal, team/group leader, institution representative). People with no current affiliation: If you wish to participate in the Challenge, your institutional official will need to sign your Data Use Certificate. If you are currently without an institutional affiliation, we suggest you ask a previous academic advisor, legal representative or a recent collaborator to serve as signing official and include a brief explanation of your circumstance when submitting the DUC. The Challenge administrators will review each request on a case per case. Access to the ROS/MAP data is controlled by the following Terms of Use: Research Use * You agree to use the Data exclusively for the purpose of participating in the Challenge. * You further acknowledge that all use of the Data shall be consistent with federal, state, and local laws and any relevant institutional policies/procedures for using and handling data. Confidentiality and Data Security * You agree to destroy all local copies of the Data as soon as the Challenge has been concluded. * You agree to keep the Data confidential and not to distribute it in any form to any entity or individual other than to those persons working on the above study who have signed a Data Use Certification, subject to applicable law. * You agree not to use the Data, or any other information, to identify or contact individual participants from whom Data were collected. * You agree to retain control over the Data and follow appropriate data security protections, ensuring physical security and information technology security of the data, and appropriate training in research on human subjects for all Challenge participants that have signed this document. * You agree that if you change institutions, you will submit a new Data Use Certification signed by an authorized official from your new institution. Dissemination of Research Results and Acknowledgment * You agree not to seek patent, trade secret or other intellectual property protection of any data or findings that result from your participation in the Challenge. * You are strongly encouraged to publish your results in peer-reviewed journals but agree not to do so until after the Challenge team has published a description of this Challenge and the winning model. * In recognition of the effort that the Data contributors have made in obtaining the Data, should you wish to publish your results, based in whole or part on an analysis of the RADC Data, you agree to follow the Data Contributor?s instructions (copied below) pertaining to proper publication review and acknowledgement. RECIPIENT will inform the Rush Alzheimer?s Disease Center (RADC) Repository of results of research using the Study Data, in confidence, by providing the RADC Repository with a draft manuscript describing such results. If RECIPIENT?s Principal Investigator desires to publish or present such research results, then, not less than thirty (30) days prior to submission to a third-party, RECIPIENT will provide the RADC Repository with a copy of any manuscript or abstract disclosing such research results for the purpose of protecting the Study Data and intellectual property of the RADC Repository that might be disclosed by such publication. In any such publication or presentation of research results, RECIPIENT agrees to acknowledge that the Study Data as follows: >"Study data were provided by the Rush Alzheimer?s Disease Center, Rush University Medical Center, Chicago. Data collection was supported through funding by NIA grants P30AG10161, R01AG15819, R01AG17917, R01AG30146, U01AG32984, the Illinois Department of Public Health, and the Translational Genomics Research Institute." * You also agree to include the following acknowledgement on all publications and presentations: ?The Datasets used for the analyses described in this manuscript were obtained as part of the Alzheimer?s Disease Big Data DREAM Challenge #1 through syn2290704. Data requestor and Signatory Official * You agree to abide by the guiding principles for responsible research use and data handling within the Synapse Commons Platform as described in the Synapse Governance documents and by the DREAM 9 Challenges Official Rules * You and your Institutional Official certify your agreement to the principles, policies and procedures for the use the Datasets as articulated in this document. Challenge Participation Eligibility Use of the ROS/MAP data is open to all individuals fulfilling the terms of use as described above, with the exception of individuals that have had previous access to the data through the Rush Alzheimer?s Disease Center (RADC), or any third party. In order to prevent an unfair advantage by having all, or most of the data from the ROS/MAP study, we will verify previous data access with RADC. Utilizing any part of the ROS/MAP data other than what is released for this Challenge will lead to disqualification.
